Payment Policies
Gutrich Chiropractic accepts major medical insurance and has a professional medical biller on staff. However, patients are responsible for knowing their policies and upholding their agreement with their insurance company. Remember, your insurance company works for you. Some insurance providers require additional paperwork to be filled out initially or on subsequent visits for compliance purposes.
At present, we are in-network providers for Blue Cross Blue Shield, United Health Care, and ACN Network. Gutrich Chiropractic works with other insurance providers as well and is happy to obtain a quote of benefits for you. We will need a copy of your insurance card and your birth date. A quote of benefits is not a guarantee, but is a general quote or estimate. This cannot be confirmed until the first claim is submitted and paid which is typically is 30 days from your first appointment.
Each patient is responsible for their contracted deductible amounts, co-pays and coinsurance at the time of service. Prepay of co-pays is accepted. We accept Mastercard and Visa, personal check or cash. When using a credit card, we require a $20 minimum charge.
We require copies of your ID and credit card to be kept on file with our patient payment agreement.
For your convenience, we will work with you to establish a payment plan to help you meet your deductible. After the thirty day period, Mandy or Stacey may talk to you again about the breakdown of benefits and any outstanding balance or credit.
On occasion, insurance payment may have been misrouted and a check may be incorrectly issued to you from your insurance company. Please endorse it to us and bring or mail the check in with its attached explanation of benefits. We intend to avoid any mishap with appropriate “assignment of benefits” paperwork which clarifies payment entitlement. However, mistakes do happen. Any remaining unpaid claims will become your responsibility.
Medicare Policies
We do accept Medicare patients except HMO Medicare. We must follow appropriate guidelines which are strict and non-negotiable. It is designed to protect all involved and to make sure treatment is medically necessary. At the time of treatment, you will be charged for any physical therapies that are not covered by Medicare. Medicare will cover 80 of CMT (Chiropractic Manipulative Treatment). You or your supplemental or secondary insurance is responsible for the remaining 20 as mandated by law.
Cash Policies
If you do not choose to use your insurance or if you do not have insurance, we do accept cash, check, or credit card for payment of service at the time of service. On occasion, if necessary, a financial arrangement can be created so you can get the treatment you need.
Personal Injury
Gutrich Chiropractic is accustomed to working with patients in accidents claimed through Worker’s Compensation or personal injury coverage. Should you be involved in this kind of incident we are happy to follow through with the legal paperwork. We trust that you will settle any debts with us upon receipt of your settlement including any charges not covered by insurance.
